Setting the scene

The Spanish flu pandemic peaked in the fall of 1918, during the deadliest wave of the deadliest pandemic in modern history. The H1N1 influenza virus infected one-third of the world's population (500 million people) and killed an estimated 50-100 million—more than World War I. The pandemic began in early 1918, likely in Kansas or China, and spread via troop movements. The first wave in spring 1918 was mild, but the second wave in fall 1918 was extraordinarily lethal, killing healthy young adults within hours through cytokine storms that overwhelmed immune systems.

What happened

The flu was called 'Spanish' because Spain, neutral in WWI, reported it freely, while belligerents censored news. The pandemic peaked in October-November 1918, coinciding with the war's end. Cities closed schools, churches, and theaters; mandated masks; and banned public gatherings. Philadelphia held a Liberty Loan parade in September 1918 despite warnings, and within days, 4,500 died. The virus spread globally, reaching remote Arctic villages and Pacific islands. In some places, mortality reached 20%.

Why it still matters

The pandemic ended in 1919, likely because the virus mutated to a less lethal form and survivors developed immunity. It killed more people in a year than the Black Death killed in a century. The Spanish flu demonstrated the vulnerability of modern societies to infectious disease, led to advances in epidemiology and public health, and influenced the creation of national health systems. It was largely forgotten until COVID-19 revived interest. The pandemic's scale—killing 2.5-5% of humanity—remains a warning about pandemic preparedness.
